Mesothelin is a glycosylphosphatidylinositol-anchored cell surface protein encoded by the MSLN gene and normally expressed on mesothelial cells lining the pleural, peritoneal, and pericardial cavities. The protein localizes predominantly to the plasma membrane and is synthesized as a precursor molecule that undergoes proteolytic processing to generate a membrane-associated mature mesothelin fragment and a soluble fragment known as megakaryocyte potentiating factor. Under physiologic conditions, mesothelin expression is relatively restricted to mesothelial surfaces, with limited distribution in most other normal tissues. Increased expression has been reported in several epithelial malignancies including malignant mesothelioma, ovarian carcinoma, pancreatic ductal adenocarcinoma, and subsets of lung and gastric carcinomas.
